Medical Exams and Forms

You are required to have a physical examination from a doctor to fill out the "Physical Examination Record for Foreigner" (Q2) form issued by the Chinese visa office if you intend to study in China for more than six months. Students intending to study in China for two or more semesters should submit the above form when applying for a Chinese visa, which is available online from the Chinese embassy website or on the Download Forms page of the China Center website. Students who have already studied for six months in China and decide later to continue for a third semester without leaving the country will be required to submit the Certificate of Verification (for physical examination record for foreigner or overseas Chinese) from the China Entry-Exit Inspection and Quarantine Department to the Public Security Bureau visa office together with your invitation letter from Zhejiang University, your passport and visa photographs. Please arrive before 8:00 A.M. and on an empty stomach with all your paper work prepared in advance (including the basic application form available from the Foreign Affairs Office, four two-inch visa photographs, and your actual passport) to the Zhejiang Provincial Health Inspection Department (see address below), since the number of applicants is large and the tests time consuming. If there are no complications you should be able to pick up your results the same day after lunch (typically 2:00 P.M.). You will be tested for an entire range of disease and illnesses and for general physical health, including a blood test for HIV, and X-ray, ultrasound and electrocardiogram (EKG) tests for cardiovascular and other diseases. Please be warned that if you do not arrive before 8:30 A.M. you will not be able to complete the exams and pick up the results the same day, and if you do not arrive with an empty stomach you will need to return another day to complete the medical exam. Applicants with visas expiring the same or following day will most likely not be given privilege over other applicants.
