Advisement Forms

Student Expense Claim
Students wishing to apply the designated portion of the Center Fee to an independent study project, internships or the Junior research Project during spring semester may petition his or her Faculty Adviser to apply these funds toward educational activities within China that facilitate a better understanding of Chinese culture. This form may also be used to claim other authorized student expenses.

Independent Study Project Proposal
Third semester students and second semester junior students may design their own independent study project (ISP), internship or Junior Research Project, within certain cultural and linguistic boundaries, and in consultation with their Faculty Adviser and/or Field Adviser. First semester Junior students and second semester sophomore students with sufficient preparation may also design an ISP based on library research, but not based on field work or an internship, unless sufficient cultural and linguistic background can first be demonstrated and with an approved petition to the Academic Director.

Emergency Contact
This form is used in case of emergency and will be given to you on the first day of the Orientation Period. Evaluation Rubrics
It is important for students to have a clear understanding of how they will be evaluated. For this reason we make our rubrics used to evaluate your writing and Portfolio of Learning, the mid-term student evaluation form, and the final Oral Presentation Evaluation form available to you in advance. In addition, we provide bilingual rubrics that you should present to your language and elective instructors two weeks prior to the last day of class. It is the student's responsibility to provide these forms to all Zhejiang University and adjunct instructors, to pickup the evaluation from each instructor and submit to your Faculty Adviser by the last day of the Long Island University semester.

Student Learning Plan
The Student Learning Plan is an agreements between the student and his or her Faculty Adviser, representing a mutual understanding between student and adviser of the courses to be taken, credits requested and requirements for full credit. The first Learning Plan is due to the Faculty Adviser by the end of the second week of classes each semester.

Community Meeting Minutes
Community meetings between students and faculty is an important and integral part of maintaining a transparent and harmonious learning community at the China Center. Community meetings are to be held once per month or as needed, and may be called by any student or faculty member at any time. Each meeting will begin with a moment of silence, and followed by an election of a student facilitator and a minutes keeper for that meeting. The minutes keeper will send a digital copy of the minutes at the conclusion of the meeting, and within twenty-four hours, as a record of the important points discussed and resolutions made.

Cash Advance and Expense Claim
Faculty who need to receive a cash advance for program expenses should request funds directly from the Cashier using the Faculty Cash Advance Record and should submit receipts for cash advances and other expenses using the Faculty Expense Claim Form.Last updated on Dec 05, 2005
Writing & Portfolio Rubrics
The faculty should use the following analytic rubrics for the holistic scoring of writing and research for all Area Studies assignments and the Portfolio of Learning. The below forms contain fields for scoring and automatically calculate the student's final score. This page should be printed (or sent via email) and returned to the student along with his or her work and your comments.

Center Evaluations
The China Center faculty, administrative staff, courses and field trips are evaluated for quality through both internal and external means.
Students will have an opportunity to evaluate the China Center courses and student services prior to mid-semester and again at the end of the semester.

The Field Adviser packet is provided to general guidelines to adjunct field advisers about her or his responsibilities and remuneration and to introduce the important role field advising plays in the Friends World Program educational model. Permanent Student Record
The Permanent Student Record narrative evaluation form should be used by the Faculty Adviser to evaluate the Portfolio of Learning at the end of each semester. A digital copy of the form should be sent to the student and to Academic Director one week after the last day of class.
